I think making it too realistic - i.e. getting tired, passing out in heat etc will take away alot of the fun. I think the realism should be restricted to the environment and vehicle physics. Maybe some destructible buildings, which of course get rebuilt when you next visit the place. And please no deserts.........please.

What if there were more than one characters to use (and develop) So different missions require different characters, eg. if one was a female, you could use her to infiltrate the police station and use her womanly wiles to get past the officers in order to obtain the report filed on the other character who has been under surveilance.... Just an idea - obviously though you wouldnt want more than two main characters or else it will become confusing and detract from the game.
