Usually accident that happen after car crashes etc like getting blown into a building and being trapped, getting trapped in a car when you're halfway getting out and you cant move, i tried putting satchel charges down to blow myself up and they wouldn't work, i had to reset the game and start from my last save, thats happened twice.
