I like the idea but it just wouldn't sound like GTA. GTA really isn't about helping cops, is it? Yes, gaining respect from cops is great but in the process, it's just mainly helping cops.

The problem with girlfriends counting toward 100% is that in a real life way, there's a chance that girlfriends will break up with you and once that's done, there's no going back, and in GTA, there's a chance you can kill them and that'll also kill your chances of getting 100%, which is why they weren't required for 100% in San Andreas.

Yes, it's a serious glitch in the mission Madd Dogg where he jumps off before you have a chance to save him. The main cause for the glitch is using the Peds Riot cheat. I'm not sure about any other cheat but the only way to get around it is to start a new cheat-free game.

There's also an army base in the K.A.C.C Military Fields in Northeastern Las Venturas but it can only be entered by a plane or helicopter outside of the mission Up, Up, and Away and during the mission but outside of the mission there's nobody in there as far as I know.
