That's done by changing textures and coding already existing files. You don't actually edit the eXE file. I'm not even a modder and I know this.

I use Photoshop 7 (old as fuck). A lot of people use CS3 or even PaintShop. Pretty simple really.

Rockstar were attempting to distance themselves from any old characters or landmarks from previous games - which was quite a good idea in my opinion.
